Your Health Information Rights

You have the right to request a copy of your health information, ask us to correct information you believe is inaccurate, and request restrictions on how we use or share your information. You may also request a list of disclosures we have made. To exercise any of these rights, please contact us using the information at the bottom of this page.

How We Protect Your Information

We implement administrative, physical, and technical safeguards to protect your protected health information (PHI) against unauthorized access, use, or disclosure. Access to patient information is limited to staff members who need it to provide or coordinate your care. We do not store payment information on our servers — all payment processing is handled by Stripe, a PCI-compliant third-party service.

Uses and Disclosures

We may use and share your health information to provide treatment, coordinate care, and process payment for services rendered. We may also use your information for healthcare operations such as quality review and staff training. We will not use or share your information for marketing purposes without your written authorization. In limited circumstances, we may be required to share information as required by law, including to public health authorities or in response to a court order.

Your Rights

You have the right to file a complaint if you believe your privacy rights have been violated. You may file a complaint with us directly or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint. You also have the right to receive a paper copy of this notice upon request, even if you have agreed to receive notices electronically.
